Individual Care Blue PlusSM
Comprehensive coverage (including prescription drugs) to protect you and your family!
Flexible Blue IISM (1500, 2500, 5000 deductible options)
Combine a comprehensive health plan with a tax-advantaged health savings account with your choice of three deductibles to meet your families needs.
Young Adult PPOSM
Basic health coverage for single young adults 18 to 30 years of age.
Flexible Blue Dental PlusSM
- Annual Maximum of $800
- 90-day waiting period for Class I and Class II services
Keep Fit
A competitively price, multiple-deductible BCBSM PPO option geared toward healthy adults 40 to 64. This plan offers financial rewards - diminishing deductibles and a fitness reward - for subscribers and, if applicable, their spouses who meet the required health criteria.
Smart Select
A medically underwritten BCN HMO product with several deductible levels to choose from, including a separate maternity deductible. This product is targeted to under age 64 individuals and families (under 19 pre-existing is waived) and priced to appeal to a wide range of individuals.
Who might consider MyBlue?
Part-time or seasonal workers who don't meet group eligibility, employees in a group's eligibility waiting period, individuals not eligible for group coverage, people in-between jobs, college students, and early retires (under 65).

Chamber Members Get BCBSM Value!
Chamber member groups will now receive Advantage Pricing with your next BCBSM renewal. Advantage Pricing offers you a rate reduction on your BCBSM medical premium when prescription drugs and/or dental benefits are also offered to your employees. Every current and new BCBSM group qualifies for Advantage Pricing. Save additional premium dollars from 4% to over 10% with new co-pay options available on all Community Blue PPO plans.
Office Visit Co-Pay options ($10, $20, $30, $40) for Community Blue PPO groups are always the same for your regular doctor, specialty care, and urgent care visits, as well as spinal manipulations.
The standard emergency room co-pay for Community Blue PPO is $50. Two new co-pay options for emergency room coverage: $100 & $150. The ER co-pay is waived if admitted and/or for accidental injury -a unique true Blue value!
Chiropractic visits for spinal manipulations are covered through Community Blue PPO with the same office visit co-pay as any office visit. Groups can opt for either 12 or 24 visits per member per year (excluding the Community Blue PPO Plus Plan 17).
Annual dental and vision exams are embedded in the new Community Blue PPO Plus Plan 16 and Plan 17 plans. Both plans offer a standard annual dental exam, x-rays, and cleaning benefit along with a standard annual vision exam ($5 co-pay) plus great VSP discounts.
The Blues continue to offer you a choice on your prescription drug programs. Choose a Two-Tier standard open formulary plan or one of four Managed Pharmacy Packaged Plans for real cost savings. The managed packaged plans use the Blues Custom Formulary with cost containment features to manage treatment costs to keep your premium dollars more affordable.
